DISCIPLESHIP FAITH FORMATION PROCESS

The Discipleship Faith Formation Process at Our Lady of Perpetual Help is a 12-year process beginning with first grade through sixth grade, which focuses on preparation of Sacraments, and continuing through twelfth grade to make life long missionary disciples of Jesus. Youth Group will be offered for middle and high school students that have received the Sacraments of Initiation.

We expect everyone to enter from first grade and continue on through twelfth grade. Please understand that if a child leaves the process after First Communion and returns later for Confirmation, he/she must do a year of catch-up preparation before placing him/her in the appropriate grade.

To register:

  1. Fill out the registration form (below) that applies to your child.
  2. Send a copy of the Baptism Certificate / First Communion Certificate to faithformation@olphkc.org. You will need these certificates to complete the online registration. If you do not have your certificates with you, please contact the parish office where your child received the sacraments to request a copy of the certificates.
  3. After completing the online registration, make a payment here:

First Reconciliation & First Communion

The heart of Christian life is the celebration of the Holy Eucharist. Baptized Christians are fully joined to the Body of Christ and His Church in the Eucharist. Discipleship Faith Formation (DFF) Classes for First Communion Year 1 are offered to first graders and are focused on First Reconciliation. First Communion Year 2 is offered to second graders who have already completed an approved First Communion Year 1 class and are prepared to receive the sacrament.

Transition Year: First Reconciliation Year 1 and First Communion Year 2:
For students entering second grade (2024-2025 academic year) who need both Sacraments
First Reconciliation Year 1:
For students entering first grade (2024-2025 academic year) who are beginning preparation for the Sacraments of Reconciliation and First Communion
First Communion Year 2:

Confirmation

The process of preparing for the Sacrament of Confirmation spans two years. In the first year, fifth graders are introduced to the sacrament through Confirmation Year 1 classes, where the focus is on the study of the sacraments and Catholic doctrine. In the second year, sixth graders participate in Confirmation Year II classes, which place a deep emphasis on the study of Christ (Christology) with a particular focus on the Holy Spirit and the Sacrament of Confirmation.

Transition Year: Confirmation Year 1 & 2 for Grades 6-12:
For students entering sixth grade through twelfth grade (2024-2025 academic year) who need the Sacrament of Confirmation
Confirmation Year 1:

Non-Sacramental Years

3rd grade: Holy Family Year
This year follows reception of First Holy Communion to deepen the family experience of this grace. The family is invited to follow the model of the Holy Family and to create a plan for this. “Do-it-yourself Family Retreat”: This family retreat can be done at home, or in parish groups using provided materials. The family will learn about their unique mission to discern each member’s role. This retreat includes preparation for a simple consecration of their home to the Holy Family.

4th grade: Ten Commandments Year
This year will facilitate further reflection on how the Ten Commandments are God’s recipe for a wonderful life of freedom. Parishes and schools will have several ways to equip parents for conversations about living the Ten Commandments.

7th–8th grade: Works of Charity and Mercy Years
Support will be provided for parents to continue having fruitful faith conversations at home. This will also include family “Action Guides.” Focus will be establishing spiritual family life in addition to attending Mass through family service projects and faith-sharing resources for parents to help develop the “language of faith” in the home.

9th–10th grade: Virtue and Morality Years
These years will emphasize the role of parents as mentors who continue to support the development of their child’s moral life and character. “The goal of the virtuous life is to become like God” - St. Gregory.

11th–12th grade: Discover Your Vocation Years
To prepare youth to venture out into the world, parents will seek to strengthen family sacramental life and day-to-day disciplines in prayer and reading of scripture.
